If you’re a kidney disease patient or a loved one, you know how difficult it can be to manage multiple health conditions, prescriptions and doctors. That job is made harder when doctors and specialists aren’t talking to each other to make sure they deliver the best care. All patients should have access to integrated care in which health care providers share information with one another in real time about a patient’s health, treatments prescribed and services delivered.
Now there’s a bill in Congress to make that happen. The Dialysis Patient Access to Integrated Care, Empowerment, Nephrologists and Treatment (PATIENTS) Demonstration Act would establish a 5-year program to make sure Medicare recipients with end-stage renal disease (ESRD) receive coordinated care.
